New forms to be used from 01 July 2011

As part of amendments to the Mining Act 1971, all forms were updated on 01 July 2011.

The new forms are available below.

Forms in the previous format are no longer valid and cannot be accepted after 01 July 2011.

eForms

eForms are MS Word documents (.doc) that can be filled in electronically and saved by clients, for regular and/or repeated use. eForms are progressively being introduced across the most commonly-used forms.

To use eForms, click on the eForm number to open the MS Word document, save it to your local drive, and then open it directly in MS Word.

If you are unable to enter text on the form, you may need to adjust your Macro or Security settings in MS Word (see sample at right) and then close and reopen MS Word, to use the form functionality.

In larger text boxes, press Shift+Enter to insert a line break.

eForms should still be printed out and signed before lodging or serving.
