National Sheep Health Statement
Complete all mandatory sections (Sections 1, 2, 3 & 5) and sign:
Section 1 - Consignment Information
- Includes owner details and address from where the journey commenced, including the Property Identification Code.
- Don’t forget, sheep born after 1 January 2006 must be identified with an NLIS tag (all sheep must be tagged after 01/01/09).
Section 2 - Ovine Johne’s Disease
- The method to calculate the Total ABC Score is explained inside the front cover.
Section 3 - Footrot
- Footrot refers to Virulent Footrot. Any condition that resembles footrot must be assessed by a veterinarian or an inspector of stock before it is classified as not being virulent footrot.
- Sheep affected or suspected of being infected with virulent footrot must not be offered for sale or transported in SA without the prior written permission of an inspector of stock.
Section 4 – Other Health Information
- This section provides the opportunity to provide additional health related information such as treatments (drenches, vaccinations and external parasites), sheep lice status of the consigned sheep and whether the sheep originate from an ovine brucellosis accredited free flock.
- Whilst completion of this section is optional, it must be remembered that sheep infested with lice must not be presented at a public market in South Australia.
Section 5 – Declaration
- This section must only be completed by the owner or person responsible for the husbandry of the sheep in the consignment.
- Original to accompany stock, duplicate remains with the vendor.
